Observation

2b) window shades have pull chains that are not securely fastened, creating a loop 2c) cords and straps are accessible to children 3a,c) broken toys, splintered wood, broken baskets, and a broken wooden tray were seen, broken down spout leaving the big playground, the infant playground has protruding nails and bolts at the fence. 3e) the playground surface is uneven and the trim along the pathway is raised 3f) cabinets are not secured to the walls to prevent tipping 3g) flaking paint throughout the facility is senn as well as broken toys 4b) windows in the classrooms were opened and did not have preventative measures in place to keep them from opening greater than 3.5 inches. 5b) outlets near sinks are not GFCI and Tamper Resistant type. 5d) Electrical cords were plugged into outlet extenders, not directly into the outlet.
